Collect and verify
Collection forms
Build forms your customers actually finish.
The wizard experience
TrustFuel forms are multi-step: one question per screen with a progress bar. Customers pick a star rating, choose how they want to share (write, video link, or screenshot), tell their story, leave their name and email, and give explicit consent. Enter advances to the next step.
What you can configure
- Headline, intro and thank-you message, edited with a live preview
- Fields: star rating on/off, video links on/off, screenshot upload on/off, require email
- Consent text, every submission includes explicit permission to display it publicly
- Active toggle, pause a form without deleting it
Welcome video (optional)
Turn on Welcome video in the form editor and paste a YouTube, Vimeo, Wistia or Loom link. It becomes the very first step of the wizard, so you can greet your customer on camera and explain exactly what to do before they start. The video tries to autoplay on load, and you can add an optional heading above it. Toggle it off anytime.
Form language
Each form has its own language setting (currently English and German) in the form editor. It switches all system texts of the wizard, buttons, labels, hints. Your own texts (headline, intro, thank-you, consent) stay exactly as you wrote them; when you switch the language while the defaults are untouched, matching defaults in the new language are filled in for you.
Limits
Starter includes 3 forms; Pro is unlimited. Screenshots and profile pictures can be uploaded straight from a phone, TrustFuel automatically resizes and converts them to a small WebP so your pages stay fast. Video stays on YouTube, Vimeo, Wistia or Loom, we never re-host your customers’ videos.
Spam protection
Every public form is protected by Cloudflare Turnstile and rate limiting. Bots do not reach your inbox.
Custom questions
Add up to 8 of your own questions in the form editor, each becomes its own step in the wizard (for example “What was your biggest win?” or “What almost stopped you from buying?”). Answers are optional for the customer and attach to the testimonial, you find them on the testimonial’s detail page. Great raw material for case studies.
Gifts: reward completed testimonials
Turn on Give a gift for completed testimonials in the form editor:
- Gift title: shown as a teaser on the form’s welcome screen (“Complete the form and you get: …”) and in the email.
- Gift URL: after a successful submission the customer automatically receives a branded thank-you email with an Open my gift button linking here. Use a PDF link (Google Drive), a course access page, a community invite, anything with a URL.
- Circle access group (optional): connect your Circle Admin API token under Settings → Integrations → Circle, then simply pick the group from the dropdown on the form (TrustFuel loads your access groups, or space groups on classic plans, live from Circle). The customer’s email is invited to your community and added to that group automatically.
The gift email needs the customer’s email address, so keep Require email address enabled on forms with gifts. Gifts are delivered right after the customer clicks the confirmation link in the double-opt-in email (see the verified reviews article for the consent records behind this).
Digital signature (optional)
Enable Collect a digital signature in the form editor and the wizard gets a signature step: customers draw their signature with finger or mouse. The statement above the pad is prefilled (you can adjust the wording per form) and both statement and signature are printed on the consent certificate, making the document look and function like a properly signed release form for your marketing.
Profile picture
Collection forms can ask for an optional profile picture (toggle Profile picture upload in the form editor). A real face makes a testimonial far more persuasive; the image is attached to the contact and shown wherever the testimonial appears. PNG/JPG, max 1 MB.
Proof of purchase
Enable Require proof of purchase in the form editor and reviewers must attach an invoice or receipt (PDF or image, max 5 MB) to submit. This is stored privately (never shown publicly) and you can view it from the testimonial’s detail page via a short-lived secure link, your evidence that a review came from a real customer.
